How to Prepare for Your Appointment
We know how valuable your time is and we want to make your experience as painless as possible.
If you are already a patient and have not yet activated your CareCloud “Breeze Portal,” please let us know and we will send you an invitation. With CareCloud, you can now pre-register for upcoming appointments, check your records and even request a new appointment.
Information For New Patients
We understand that our new patients can feel a bit overwhelmed with HIPPA and other medical paperwork, so we ask that you take your time to fill out all of the necessary forms in the comfort of your own home.
Our new CareCloud “Breeze Portal” helps you pre-register, fill out the required forms and update your information from your phone or preferred device. If you have any difficulty or have questions about paperwork and forms, please feel free to contact us and we will be happy to walk you through the process.
On the day of your appointment, a medical assistant will confirm all of your information once you’ve been checked-in. Once we’re certain we have all of your information correct, you’re all set for your appointment!
If you are unable to complete your forms prior to your appointment, please do not worry. We just ask that you arrive 20 minutes before your scheduled time and we will get everything taken care of.
Pre-Registration Instructions
If you have an appointment scheduled:
- You should have received an invitation from CareCloud.com.
- Open your new appointment in CareCloud and select “Pre-registration.”
- You will be guided through each section necessary for your appointment.
- Once completed, click “Submit” and we’ll handle the rest.
If you do not have an appointment:
- Please send us an email at info@albanoclinic.com or call us at 385-220-9009 and we will walk you through the process.
- We will need your name (first and last), date of birth and email address.
Don’t have access to a laptop or cell phone? No problem – we’ve included all of our patient forms for your convenience. Please print and complete your forms prior to your appointment.
If you are able to do so, we ask that you please email your forms to us at info@albanoclinic.com. This really helps us maximize your time with your doctor. If not, that is okay too – just be sure to bring them with you!
Special Instructions
If you are coming in for one of the following types of appointments, please take note of these special instructions.
New Patient Appointment
Please bring any of the following to your upcoming appointment: medical records, X-rays or MRI reports and imaging disks. You may also have them sent prior to your appointment by either faxing them to 877-770-6252 or emailing them to info@albanoclinic.com.
FAA Exams
Please drink plenty of water, as you will need to give a urine sample. If you wear glasses when flying, you must bring them with you for your eye test.
Also, be sure to have your MedXPress paperwork completed prior to your appointment.
BHRT Blood Draw
For patients on our BHRT program who need additional blood work, please take your compounded medications five hours prior to your blood draw.
Note: you must be on your current prescription. If you are not current, please contact our office.
It is also important that you take your supplements five hours prior to your blood draw. Please suspend taking any multivitamin or B vitamins until after your blood draw, as these can affect your lab results. Drink plenty of water, as it helps with blood flow.
For the lab results to be accurate, you must be on the correct dose of your hormone medications. If you are out of your current hormone medications, please reschedule your blood draw.
We ask that you fast for 12 hours (nothing but water) if you are having your cholesterol checked.
